Rescheduling Your Appointment
CONTACT THE OFFICE BY PHONE DURING BUSINESS HOURS IF YOU ARE CANCELLING OR RESCHEDULING SURGERY.

We will not reply to requests that do not meet these terms. Ensure you understand the Office Policies which explain how patients are triaged.

This is NOT for new patients who have not yet been assigned an appointment. We will NOT reply if you are not in our system.
This is only for patients who are already scheduled (we have sent you a notification) OR are due for appointments when we have asked you to contact us. We will check to documentation that you are due for an appointment before scheduling. You may need to request a new referral from you family physician.
If you have not been asked to contact us for an appointment or have not been referred, please ask your family physician to make a new referral.
We acknowledge receipt of referrals to the referring doctor once they are processed. If you do not already have an appointment you can check with your family physician that we have received and accepted the referral or wait for us to assign an appointment.
Wait times for conditions which are not identified as urgent may be more than 6 months.

Due to high demand and pre-existing commitments (e.g. surgery) your new appointment will be assigned a later date. In some cases your appointment may be delayed by several weeks.
We understand that some patients may wish to be seen sooner than their assigned appointment. We have scheduled your appointment based on our availability and the relative seriousness of your condition. You may review information on how we triage patients here.
If you are a new patient and believe that you require an earlier appointment than you have been assigned OR have not yet received an appointment notification you may contact the physician who referred you. We would be pleased to speak with the referring physician or review any information they submit to justify an earlier appointment date.

We do not provide appointments on Saturday, Sunday or any statutory holiday.

We are unable to hold appointments once a change has been made. This allows us to re-assign the time to another patient. We will make every effort to reschedule your appointment as soon as possible.

Late cancellations and requests for appointment changes may be subject to additional fees. Information is available in the Office Policies section on ‘No-Show/Late Cancellation Fees’ .

If the information in your request does not match the information that was provided by the referring patient we will NOT respond. Contact information includes the PHN, date of birth and other contact information (phone number(s) or email) You should ensure that your family physician has your current contact information.
This requirement is to protect patient confidentiality.
